ALTERNATE TRANSPORTATION POLICY
If you are requesting that your child be picked up or dropped off at any address other than your residence, you must complete, sign and return a request for alternate transportation form.
If you are not familiar with the Lake Board of Education’s policy on alternate transportation, the following is a brief summary:
Forms must be received in the Transportation Office by June 15 to begin the first day of a new school year. During the school year, forms must be turned in at least
three (3) school days prior to the requested beginning date.
Changes cannot be made for less than five (5) days.
Students must be picked up at the same location every day.
Students must be dropped off at the same location every day.
Phone calls or notes from parents will not be accepted.
Students may not ride to or from school with a friend.
When completing the form, please check the following items carefully:
Include all necessary phone numbers.
Changes cannot be made without the proper signature.
Your student will be given new bus information after your request has been
processed in the Transportation Office.
